I don't know the specifics, but from what I recall Cortisone shots are just a mask, they don't actually heal the area. I've also heard that the effectiveness wears off the more you do. So use it wisely.
I need it for my shoulder (I still lift heavy like a dumbass), but I've been reluctant to start. I just adjust my workouts, manage the pain, ice, etc. I am getting to the point where it is becoming harder to manage though.

One quick tip. Burn your carbs after you lift, not before. 10 min warmup on the tready and then lift. If you need more treadmill time, add another 20-30 mins after your lift. You want to be muscles to be fresh to get the most out of a lift.
As far as what to do, what is your goal? Bigger, faster/fitter, stronger? Pick one-ish. Without getting too advanced you can't accomplish all three. Focus on one (maybe two) and alternate every 6-8 weeks if needed. Set a goal for each period. Is it a strength gain goal? A muscle gain goal? An overall fitness gain? Based on your "flailing" comment, it sounds like this is your starting point. It is easy to get lost in the maze of equipment and moving stuff around without purpose or gain.
Goals can be simple. They don't need to be "I wanna bench 450". It can be as simple as "I wanna gain a little muscle", "get a little stronger", etc.

my only advice, for the purpose of nostalgia and my love of the area would be north Georgia into North Carolina. Maybe along the trail around the Nantahala National Forest by Franklin and up to Fontana Lake.

Just FYI I am the wrong guy to ask that question. The most full-time length I have ever done is in GSMNP and it is awesome but also very popular. Too many rocks and roads in MD/PA so I wouldn't suggest that.
If I'm guessing, the stretch just prior to GSMNP (Nantahala as Bob mentioned) and the stretch after GSMNP all the way to Mt Rogers and beyond is probably similar to the best. I'd start looking there if I was looking. Great resources linked, Bob.
The AT has the cachet (and for a reason) but there are plenty of great week-long routes designable in and around GSMNP that would get less traffic and also make water access a lot easier than it is on the AT.

My knees are bad. They were so bad at one point I couldn't sleep at night for more than an hour at a time. Finally found a Dr who did cortisone shots. Yes, they mask it, but its taken the pain away, yes they have diminishing returns as well. This first one is more or less a trial to see how long it lasts, its been around 3-4 months and the pain hasn't come back as bad as it was. That being said, I really dropped off the exercise and gained back about 15-20lbs in the last year. I am a bit concerned that my knee is going to get back to being as bad as it was. Flip side, I'm sure my blood work isn't going to look that great either. Be a good example for the girls - maybe somehow work them into a workout periodically (Not all the time - you need that time for yourself, too) - but let them see a strong woman in action.

Zero chance my knees and back would let me do that. I moved ten yards about six years ago, but wisely did it over three days and my back still hated me.

I've been going to Spin in Old Town since Hunter closed Evergreen on Grand River. Spin is a pretty good store and I've been impressed with the owner. I think his name is Chad.
I used to go to Velocipede but that closed a few years ago. Bought my first real bike there in 9th grade. Before that it was kids bikes or hand-me-downs. I think the brand of the bike was called SR.
